Experience
24 months of role experience is required with some in the last 12 months.
24 months of Home Health experience is required with some in the last 24 months.
References
Candidates must provide 1 manager/supervisor reference from within the past 12 months in order to be considered for this role
Candidates must provide 1 reference from within the past 3 years in order to be considered for this role
License
Candidates must have a license

Orientation Hours: 12 Hours

Schedule: M-F

Coverage Areas: South King (Burien, Tukwila, Kent, Federal Way) and Pierce County (Tacoma, Lakewood, Puyallup)

Common diagnosis / Types of patients/patient demographics Primarily Geriatric

Experience or 'Must have' skills Will be working with COTA's

How many patients do they see per day? 4-5 depending on visit type

Primary focus with be Evals and OASIS time points

Home Health/Hospice Aids? HHA

Holiday Requirement: No

Is there a call or standby requirement? No

Floating Requirements May need to see patients in boarding geographic area if their assigned area is low

Scrub Color/Dress Code Scrubs ok otherwise casual but professional
