Requirements

Experience

12 months of role experience is required with some in the last 12 months.

Must have at least 24 months of Home Health experience.

3 months of experience with Epic is required from any number of months.

Experience with Oasis is preferred from any number of months.

References

Candidates must provide 1 manager/supervisor reference from within the past 12 months in order to be considered for this role

Candidates must provide 1 charge/lead reference from within the past 3 years in order to be considered for this role

License

Candidates must have a Wisconsin license or compact license

Certifications

BLS (Required)

Vaccinations

COVID vaccination required. Religious and medical declinations accepted.

Additional Details:

An interview with the unit manager may not be available prior to offer. If you do not interview with the unit manager, you may interview with a third-party clinical nurse manager.

Required Skills/Experience: Point of care documentation experience

Required Credentials: Graduate from an accredited school of nursing

Preferred Credentials: BSN

Shift & Scheduling: Weekend /Holiday Rotation Requirements: Every other weekend (open to every weekend). Up to every other holiday.

Floating Requirements: Travel from home to home for patient care. covering a 9 county radius in NE WI (average travel of 75-100 miles per day, 4-6 visits per day with travel time included in shift length) Must have own reliable vehicle
