Requirements

Experience

12 months of role experience is required with some in the last 12 months.

Travel experience is required from any number of months.

Experience with Oasis is required from any number of months.

References

Candidates must provide 1 charge/lead reference from within the past 3 years in order to be considered for this role

Candidates must provide 1 manager/supervisor reference from within the past 12 months in order to be considered for this role

License

Candidates must have a Wisconsin license or compact license

Home Health /Case Management

Guaranteed Hours Per Week: 38 with flex to 40 hours per week

Weekend/Holiday: Weekend rotation is every 3rd- 4th weekend currently. The range is every 4-6. holiday rotation

On-call: 1-2x during the week and every 3rd or 4th weekend (at least one per month)

Floating Requirements: at times, when there is a low patient census, possibility of floating to other territories to assist when needed

Minimum experience: 2+ years home health experience, preferred

Charting Experience: Oasis required. Epic preferred.
