Requirements
Experience
12 months of role experience is required with some in the last 12 months.
Must have at least 12 months of Case Management experience.
BSN completion preferred
References
Candidates must provide 2 manager/supervisor references from within the past 3 years in order to be considered for this role
License
Candidates must have a District of Columbia license

Discharge Planner Case Manager

Required Skills/Experience:

  • Skills required: Emergency Department, Case management/Utilization Review, Care coordination, Discharge Planning, Work Settings, Case management, Documentation/Communication, Discharge Summary, Documentation according to plan of care, Documentation of care delivery, plan, goals, etc, Documentation of patient/caregiver teaching/learning, Documentation reflects understanding of coverage criteria, Documents patient response, Education, Educates patient/caregiver to facilitate discharge, Provide / reinforce individualized patient/caregiver education, Acute Hospital

Preferred Skills/Experience:

  • Previous Charge experience.
  • Community Hospital experience.
  • LTAC hospital experience.
  • Skills: Home Healthcare, Hospice, Long Term Acute Care (LTAC), LTC/ Nursing homes, Rehab, Short Term Acute Care Inpatient Medical Social Worker, Medical Surgical unit

Required Credentials: Will accept pending DC Licensure with proof of application.

Special Requests: RTO must be on the submittal and cannot be changed once the profile has been sent to the manager for review.

Shift & Scheduling:

  • Schedule is Friday to Monday.
  • Weekend Requirement: weekends included in rotation every 7 weeks; holiday coverage as needed

References must cover at least 1 full year of relevant experience in specialty within the past 3 years.

Manager may or may not interview. Clinician must be okay with this. If Clinician has questions, please email the AM.
