Experience

24 months of role experience is required with some in the last 12 months.

Must have at least 12 months of Physical Therapy experience.

References

Candidates must provide 1 manager/supervisor reference from within the past 12 months in order to be considered for this role

Candidates must provide 1 reference from within the past 3 years in order to be considered for this role

License

Candidates must have a license

Certifications

BLS (Required)

Additional Details:

Required Skills/Experience:

1 year of experience must be within a Skilled Nursing Facility.
Home health experience.

Required Credentials: Dementia testing will be required prior to start.

Shift & Scheduling: Monday thru Friday.

Unit/Facility Details: Settings: home care, hospice, and long term care - need to cover Home Health and SNF.
