Requirements

Experience

12 months of role experience is required with some in the last 12 months.

24 months of Home Health experience is preferred from last 12 months.

Experience with Netsmart Technologies is preferred from any number of months.

References

Candidates must provide 1 charge/lead reference from within the past 3 years in order to be considered for this role

Candidates must provide 1 manager/supervisor reference from within the past 12 months in order to be considered for this role

Additional Details:

Can submit with a block scheduling request - The facility will always try and honor, but never guarantee.

Shift Requirements: Variable days (Monday- Friday)

Weekends: PT's rotate one Saturday every 6-8 weeks. (will be given a a day off during the week on the weeks they work Saturday)

Float Requirements: occasional travel to central/ Northern regions

Population served includes adult and geriatric patients.

Required Education: Bachelor's Degree in Physical Therapy

Minimum Experience:

Home Health experience
Training working with the adult/geriatric population with a variety of orthopedic, neurological and functional mobility diagnosis.
Knowledge of Physical Therapy standards and practices acquired through a minimum of a Masters degree in Physical Therapy.

Required Certifications: Valid Drivers License (with proof of car insurance)
