Requirements

Experience

12 months of role experience is required with some in the last 12 months.

12 months of Case Management experience is required with some in the last 24 months.

3 months of experience with McKesson is preferred from any number of months.

References

Candidates must provide 1 manager/supervisor reference from within the past 12 months in order to be considered for this role

License

Candidates must have a Vermont license or compact license

Additional Details:

Central Vermont Home Health & Hospice agency in need of an experienced Hospice RN:

Experience with hospice case management required.

Experience providing hospice care in a home setting required.

Prior experience with rural travel preferred.

Work schedule: Regular hours for this position are 4 days per week - Friday-Monday 8AM-5PM, and acts as after hours on-call triage and runner Friday 5PM-Monday 8AM.

Prefer a nurse with no RTO requests for this contract.
