Requirements

Experience

36 months of role experience is required with some in the last 12 months.

Must have at least 12 months of Case Management experience.

BSN completion required

12 months of Trauma Level 2 facility is required from the last 2 years

3 months of experience with Epic is preferred from any number of months.

References

Candidates must provide 2 references from any number of months in order to be considered for this role

License

Candidates must have a New Hampshire license or compact license

Additional Details:

Shift & Scheduling: Monday - Friday. No block scheduling. No weekends or on call.

Unit Details: The RN Care Manager will oversee a daily caseload of patients on the inpatient units with a variety of care needs. Will attend interdisciplinary rounds. Complete Initial assessments. Facilitates the development of a plan of care in collaboration with the patient, family, caregiver and health care team.
