Requirements

Experience

24 months of role experience is required with some in the last 12 months.

Must have at least 12 months of Case Management experience.

Experience with Epic is preferred from any number of months.

References

Candidates must provide 2 references from within the past 2 years in order to be considered for this role

License

Candidates must have a New York license

Additional Details:

Required Skills/Experience: 1 year Medical/Surgical and/or Community Nursing, or prior experience in acute care discharge planning experience. Familiarity with discharge planning/UR process and community agencies. 2 years experience required for non U.S. candidates.

Preferred Skills/Experience: Certification for PRI, Screen and MDS.

Shift & Scheduling: Mon-Fri. 40 hours per week. No weekends or holidays.
