Requirements
Experience
12 months of role experience is required with some in the last 12 months.
Must have at least 10 months of Case Management experience.
Experience with Epic is required from any number of months.
References
Candidates must provide 2 manager/supervisor references from within the past 3 years in order to be considered for this role
License
Candidates must have a Oregon license

Additional Details:

Cannot be currently or recently (within 6 months) employed with network - Must have left in good status, if former employee.

ER profiles must include the size of the facility / # beds and the Trauma Level designation (if any)

Orientation for all Oregon facilities is in Portland, OR for RN, LPN and CNAs.

Required Skills/Experience: Work Settings, Acute Hospital, Case Management/Utilization Review, Care coordination, Discharge Planning, Utilization Review, Regulatory, CMS: Centers for Medicare and Medicaid Services, HIPAA guidelines (Health Insurance Portability and Accountability Act), OSHA, Acute Hospital Experience by Specialty

Preferred Skills/Experience: Admission Criteria, Admission Criteria, Appeals and Denials, Continued Stay Reviews, Medical Necessity, DRG (Diagnosis Related Groups), NCQA (National Committee for Quality Assurance), The Joint Commission/ Core Measure/National Safety Goals, MS, SDU/PCU/IMC/Obs, Surgical Services

Required Credentials:

• BLS: Must be AHA (Instructor, Provider, or RQI) or “Heart&Stroke” certification from Canadian Heart & Stroke Foundation only.

• Drivers License/State ID: Government-Issued Photo ID

• Required: 2 Supervisory References covering 1 year (6 months for CNA’s) within last 3 years during the compliance process and prior to start. Can include Charge RNs/Lead.

• Cannot have any hits/suspensions on license or any background hits from any point in the past.

Unit/Facility Details: Parking Cost: No Fee

Special Requests:

• All positions at Providence Health & Services require that any and all lab items requested during onboarding be completed within 72 hours of signing a contract - no exceptions. This means candidates must be responsive and willing to go to a lab to complete health orders ASAP after signing. Failure to comply may lead to a pushed start or cancellation as assessed by the facility.

• RTO must be listed at time of submission.

• Must have a vehicle for this assignment.

Shift & Scheduling:

• May consider 10 hour shifts but would prefer 8 hour shifts.

• Weekend coverage varies by need, PTO's etc.

• No on call
