Orientation for all Oregon facilities is in Portland, OR for RN, LPN and CNAs.

Required Credentials:
• BLS: Must be AHA (Instructor, Provider, or RQI) or “Heart&Stroke” certification from Canadian Heart & Stroke Foundation only.
• Drivers License/State ID: Government-Issued Photo ID
• Required: 2 Supervisory References covering 1 year (6 months for CNA’s) within last 3 years during the compliance process and prior to start. Can include Charge RNs/Lead.
• Cannot have any hits/suspensions on license or any background hits from any point in the past.

Special Requests:
• All positions at Providence Health & Services require that any and all lab items requested during onboarding be completed within 72 hours of signing a contract - no exceptions. This means candidates must be responsive and willing to go to a lab to complete health orders ASAP after signing. Failure to comply may lead to a pushed start or cancellation as assessed by the facility.
• RTO must be listed at time of submission.
• Must have a vehicle for this assignment.
Shift & Scheduling:
• May consider 10 hour shifts but would prefer 8 hour shifts.
• Weekend coverage varies by need, PTO's etc.
• No on call
