Requirements
Experience
12 months of role experience is required with some in the last 12 months.
Must have at least 24 months of Case Management experience.
BSN completion preferred
Experience with MedConnectHealth is preferred from any number of months.
Experience with Cerner is preferred from any number of months.
References
Candidates must provide 2 manager/supervisor references from within the past 12 months in order to be considered for this role
License
Candidates must have a District of Columbia license

Case Manager/ Discharger Planner

Previous Employment Requirements: If a nurse has held previous employment with Medstar, must be gone for a minimum of 1 year to return as a traveler.

Required Skills/Experience:

  • Skills: Acute Hospital
  • Additional Requirements: Initial assessment/psychosocial, crisis management, facilitate in our disciplinary team meetings and family meetings.
  • Case Management/Utilization Review include: Discharge Planning, benefits eligibility, care coordination, conducting needs assessments and order DME; rehab placement

Preferred Skills/Experience:

  • Skills: Long Term Acute Care/Rehab/Skilled Nursing
  • Arrange Follow Up Services
  • Cerner/Novi Health for insurance updates and SNF referrals

Required Credentials: Drivers License/State ID

Unit/Facility Details:

  • The case manager will be working on the orthopedic/amputee unit in an inpatient acute rehab rehabilitation setting.

Shift & Scheduling: Weekends included in rotation (every 5th or 6th weekend); holiday coverage as needed

References must cover at least 1 full year of relevant experience in specialty within the past 3 years.

Manager may or may not interview. Clinician must be okay with this. If Clinician has questions, please email the AM.
