Special Requests: No travel pairs or trios. No scheduling requests (weekends only, block scheduling, etc). No background hits. RTO will not be approved in the first week of assignment or holidays. All facilities the traveler is open to working in should be listed on the submission. If interested in “any” location, please indlude what states (I.E. Any AZ, Any CO, Any state, etc.) All shifts the traveler is open to working on should be listed on the submission. All departments the traveler is open to working must be checked on the cover page.
Shift & Scheduling: Banner is allowed to cancel 1 shift per week. Weekend and on call schedule requirements to be determined by facility.
Floating Requirements: Must be willing to float to units where experienced.
