Requirements

Experience

24 months of role experience is required with some in the last 12 months.

24 months of Case Management experience is required from the last 4 years.

Experience with Cerner is required from any number of months.

References

Candidates must provide 1 reference from within the past 3 years in order to be considered for this role

Candidates must provide 1 manager/supervisor reference from within the past 12 months in order to be considered for this role

License

Candidates must have a California license

BLS (Required)

Areas to cover: All areas of the hospital, no OP and no Home Care.

Required Skills/Experience: Must have prior CA Dignity experience or they will not considered.

Technology Used on Unit: EMR: InterQual

Previous Employment Requirements: Candidates may not have been directly employed (full time, part time, or PRN) by any facility associated with CommonSpirit, including CHI and/or Dignity within the past year. Candidates who were employed PRN through outside firms will be considered.

Shift & Scheduling:

Weekend Requirement: every other depending on staffing. Would be at least one full weekend per month.
Holidays not required, but will work if they are willing.

Floating Requirements: First to float.

Additional Notes: This position is for both UR and discharge planning.
