Requirements
Experience
12 months of role experience is required with some in the last 12 months.
Must have at least 12 months of Home Health experience.
BSN completion preferred
Trauma Level 1 experience is preferred from any number of months
Experience with Epic is required from any number of months.
References
Candidates must provide 1 manager/supervisor reference from within the past 2 years in order to be considered for this role
Candidates must provide 1 reference from within the past 2 years in order to be considered for this role
License
Candidates must have a Virginia license or compact license

Additional Details:

Required Skills and Experience: 3 years of critical care, acute care, or medical surgical RN experience. Documentation Guidelines, Admission Assessment, Routine visit, Significant change in condition, Start of Care, Transfer, Adult Home Health, Clinical assessment documentation, Coordinate services, Infection Control Precautions, Obtain Vital Signs, Medication administration/management, Medication Administration, Medication Assessment/Management.

Preferred Skills and Experience: Experience managing acute care or med surg adult and/or geriatric patient populations. Telephone triage experience preferred. Home health experience preferred.

Required Credentials: Government issued photo ID required. Photo Identification must be carried on person during hours of service to client. Drivers License/State ID.

Shift & Scheduling Requirements: Rotating 12 hour shifts - 7a-7p and 11a-11p. Rotating Weekends and Holidays

Special Request Requirements: No time off requests during the first week of the contract.

Unit and Facility Details: Facility will review all background hits, etc.
