Requirements

Experience

24 months of role experience is required with some in the last 12 months.

Must have at least 12 months of Home Health experience.

References

Candidates must provide 1 manager/supervisor reference from within the past 12 months in order to be considered for this role

Candidates must provide 1 charge/lead reference from within the past 12 months in order to be considered for this role

License

Candidates must have a Massachusetts license

BLS (Required)

Additional Details:

Required Skills/Experience:

One year Certified Home Care experience.
One year Case Management Experience.

Shift & Scheduling:

No on-call requirement.
Weekend rotations once every 6 weeks.

Cape Ann Pod – Essex, Gloucester, Manchester, Rockport.
Ipswich Pod – Ipswich.
North Pod – Amesbury, Bradford, Byfield, Georgetown, Groveland, Haverhill, Merrimack, Methuen, Newbury, Newburyport, Rowley, Salisbury, West Newbury.
