Requirements

Experience

24 months of role experience is required with some in the last 12 months.

24 months of Home Health experience is required with some in the last 24 months.

Experience with Epic is preferred from any number of months.

References

Candidates must provide 1 reference from within the past 3 years in order to be considered for this role

Candidates must provide 1 charge/lead reference from within the past 12 months in order to be considered for this role

Additional Details:

An interview may not be available prior to offer.

Guaranteed Hours 40 hours per week

Mon-Fri

RTO needed upfront - Additional RTO will not be approved, including after submissions.

Common diagnosis / Types of patients/patient demographics - Adults, fx's, weakness, new joints, CVA follow up. Home exercise programs.

Experience or 'Must have' skills Home Health experience preferred, not required.

Floating Requirements Will be cross trained to both home health and hospice.

Scrub Color/Dress Code Teal scrubs or business casual.
