Requirements

Experience

24 months of role experience is required with some in the last 12 months.

Travel experience is required from any number of months.

Must have at least 36 months of Home Health experience.

Experience with Oasis is preferred from last 12 months.

Experience with Netsmart Technologies is preferred from any number of months.

References

Candidates must provide 2 charge/lead references from within the past 12 months in order to be considered for this role

License

Candidates must have a Vermont license or compact license

Additional Details:

Unit/Facility Details: Public Transportation is Not accessible in this area, please ensure you have reliable transportation, personal vehicle or rental vehicle are strongly recommended. Vehicles should be equipped with all season or snow tires during the months of November – April.

Shift & Scheduling: Assignment for up to 32 weeks. Cannot accommodate 10H shifts. 1 weekend per month, will have time off during week to accommodate weekend schedule. Monday to Sunday.
