12 months of role experience is required with some in the last 12 months.
Must have at least 24 months of Case Management experience.

Case Management - LCSW Behavioral Health

Shift: Monday to Friday

Experience At least one year of actual work experience in a hospital setting or an ongoing plan for consultation between the social worker and a qualified social worker must be developed. Must have knowledge of community resources to meet post discharge clinical and social needs.

Main Facility will be Valley Hospital but may need to float through the VHS Health System (Will float to all VHS hospitals- Centennial Hills, Henderson Hospital, Spring Valley, Summerlin, and Valley hospitals. (we would try to assign them to one facility for continuity although they may have to float for sick calls.)
