Requirements
Experience
12 months of role experience is required with some in the last 12 months.
12 months of Home Health experience is required with some in the last 24 months.
Experience with Homecare Homebase (HCHB) is required from any number of months.
Experience with Oasis is required from any number of months.
References
Candidates must provide 2 manager/supervisor references from within the past 12 months in order to be considered for this role
License
Candidates must have a Wisconsin license or compact license
Certifications
BLS (Required)
Proof of Identification
Driver's license required

Additional Details:
Guaranteed 40 hours per week
Shift: Monday - Friday, 8 - 5
Territory: Greater Milwaukee area. Need them to be flexible on what part of the city we have them provide care. We will work to minimize drive time, as always, but the area may change.
Must meet minimum productivity expectation of 30 points per week.
