Requirements
Experience
12 months of role experience is required with some in the last 12 months.
Must have at least 24 months of Home Health experience with 12 months within the past 3 years.
Experience with Homecare Homebase (HCHB) is required from any number of months.
References
Candidates must provide 2 manager/supervisor references from within the past 12 months in order to be considered for this role
License
Candidates must have a Michigan license
Certifications
BLS (Required)

Driver's license required

Additional Details:
Unit/Facility Details: Administer skilled nursing care and education under the supervision of registered nurse for adult clients in their place of residence, coordinate with the interdisciplinary team, patient/family and provider.
