Requirements
Experience
12 months of role experience is required with some in the last 12 months.
Travel experience is required from any number of months.
Must have at least 10 months of Speech-Language Pathology experience.
Experience with Epic is required from any number of months.
References
Candidates must provide 2 manager/supervisor references from within the past 3 years in order to be considered for this role

Unit / Facility Details: Outpatient Therapy Clinic

Transportation: Clinician must have reliable transportation; travel pairs require separate vehicles

Housing: If applicable - Clinician should secure housing before accepting and be aware of local cost of living

Proof of Identification: Drivers License/State ID, Social Security Card

Special Requests: RTO must be listed at time of submission.

All positions at Providence Health & Services require that any and all lab items requested during onboarding be completed within 72 hours of signing a contract - no exceptions. This means candidates must be responsive and willing to go to a lab to complete health orders ASAP after signing. Failure to comply may lead to a pushed start or cancellation as assessed by the facility.

References must cover at least 1 full year of relevant experience in specialty within the past 3 years

Cannot have any hits/suspensions on license or any background hits from any point in the past.

If you have a multistate/compact nursing license, you will be required to complete a one time 6 hour suicide prevention training according to the WA state regulations. (Nursing positions only).

Shift & Scheduling: Pre-Approved Time Off: 1 Request

Facility scheduling needs must be met; special requests (e.g., block scheduling, late starts) will not be accommodated

You must be able to complete labs within 48 hours of signing.
