Requirements

Experience

24 months of role experience is required with some in the last 12 months.

Must have at least 12 months of Physical Therapy experience with 9 months within the past 12 months.

3 months of experience with Cerner is preferred from any number of months.

Experience with Net Health is preferred from any number of months.

References

Candidates must provide 1 charge/lead reference from within the past 12 months in order to be considered for this role

𝗨𝗻𝗶𝘁/𝗙𝗮𝗰𝗶𝗹𝗶𝘁𝘆 𝗗𝗲𝘁𝗮𝗶𝗹𝘀: Number of Beds : 24 acute/swing beds, 40 nursing home beds, 10 skilled beds within nursing home

𝗦𝗵𝗶𝗳𝘁 & 𝗦𝗰𝗵𝗲𝗱𝘂𝗹𝗶𝗻𝗴: 40 hours per week, schedule needs to be reviewed during interview with rehab manager / SUN-THURS

𝗙𝗹𝗼𝗮𝘁𝗶𝗻𝗴 𝗥𝗲𝗾𝘂𝗶𝗿𝗲𝗺𝗲𝗻𝘁𝘀: clinician will be asked to float between acute/swing, nursing home, and out-patient services as indicated
