Requirements
Experience
12 months of role experience is required with some in the last 12 months.
Must have at least 60 months of Case Management experience.
BSN completion required
References
Candidates must provide 1 manager/supervisor reference from any number of months in order to be considered for this role
Candidates must provide 1 reference from any number of months in order to be considered for this role
License
Candidates must have a Michigan license
Certifications
BLS (Required)

Additional Details:

Required Skills/Experience: Minimum 5 years acute inpatient case management experience with experience in acute discharge planning

Previous experience using CarePort system

Preferred Skills/Experience: Candidates with previous experience charting in AllScripts, GEMS/Sunrise are preferred.

Minimum of three years RN clinical experience, preferred.

Current working knowledge of discharge planning, utilization management, case management, performance improvement and managed care reimbursement, preferred.

Shift & Scheduling: Weekend rotation required.
