Requirements
Experience
12 months of role experience is required from last 3 months.
12 months of Case Management experience is required from last 3 months.
3 months of experience with Epic is preferred from any number of months.
References
Candidates must provide 2 references from within the past 12 months in order to be considered for this role
License
Candidates must have a Vermont license or compact license
Certifications
BLS (Required)
Vaccinations
COVID vaccination required. Religious and medical declinations accepted.

Case Manager – Home Health
Graduate of an accredited school of nursing.
Medical/Surgical nursing and/or home health care experience.
Day shift - 8:00am - 4:30pm, Monday - Friday w/rotating weekends
32 guaranteed hours.
