Requirements

Experience

24 months of role experience is required with some in the last 12 months.

Travel experience is required from any number of months.

Must have at least 24 months of Home Health experience.

Experience with Oasis is required from any number of months.

Experience with Epic is required from any number of months.

References

Candidates must provide 1 charge/lead reference from within the past 3 years in order to be considered for this role

Candidates must provide 1 manager/supervisor reference from within the past 12 months in order to be considered for this role

License

Candidates must have a Michigan license

Vaccinations

Additional Details:

An interview with the unit manager may not be available prior to offer. If you do not interview with the unit manager, you may interview with a third-party clinical nurse manager.

Preferred Skills/Experience: 2+ years home health experience, preferred

Required Credentials: Valid Driver's license

Unit/Facility Details: 20-25 patients weekly, 4-5 visits per day on average. It can vary from 25-60 miles per day depending on the territory, patient census, staff availability, etc

Mileage can be up to and over 100 miles as it is a large territory

Special Requests: Can submit with a block scheduling request - The facility will always try and honor, but never guarantee.

Shift & Scheduling: Depending on census there may be flexibility to start and end times.
