Requirements

Experience

12 months of role experience is required with some in the last 12 months.

Must have at least 24 months of Physical Therapy experience with 12 months within the past 3 years.

Experience with Oasis is required from any number of months.

References

Candidates must provide 2 manager/supervisor references from within the past 2 years in order to be considered for this role

Additional Details:

RTO must be listed at time of submission.

All positions at Providence Health & Services require that any and all lab items requested during onboarding be completed within 72 hours of signing a contract - no exceptions. This means candidates must be responsive and willing to go to a lab to complete health orders ASAP after signing. Failure to comply may lead to a pushed start or cancellation as assessed by the facility.

References must cover at least 1 full year of relevant experience in specialty within the past 3 years

Cannot have any hits/suspensions on license or any background hits from any point in the past.

Required Skills/Experience: 1 year of recent Skilled Home Health Experience (at least 1 year within the last 3 years)

Shift & Scheduling: Monday - Friday

Types of Patients on Unit: Case Load: Geriatric

Shift & Scheduling: Traveler is expected to have their own transportation.

Ratios: May visit up to 6 patients per day; 30 productivity points standard
