Requirements
Experience
12 months of role experience is required with some in the last 12 months.
Must have at least 24 months of Home Health experience within the past 3 years.
Experience with Homecare Homebase (HCHB) is required from any number of months.
References
Candidates must provide 2 manager/supervisor references from within the past 12 months in order to be considered for this role
License
Candidates must have a Ohio license or compact license
Certifications
BLS (Required)
Proof of Identification
Driver's license required

Additional Details:
Required Skills/Experience: Must have at least 12 months of Hospice or Home Health experience within the past 3 years
Technology Used on Unit: HCHB and OASIS for HH
Shift & Scheduling: M-F, Every third weekend
Any call?: Yes, approximately 2-3 on call shifts per month
Floating Requirements: Territory: Columbiana, Mahoning, Trumbull County
