Requirements
Experience
24 months of role experience is required with some in the last 12 months.
Must have at least 24 months of Case Management experience within the past 4 years.
BSN completion preferred
Experience with Epic is required from any number of months.
References
Candidates must provide 1 manager/supervisor reference from within the past 12 months in order to be considered for this role
Candidates must provide 1 reference from within the past 3 years in order to be considered for this role
License
Candidates must have a Mississippi license or compact license
Certifications
BLS (Required)
CCM (Preferred)
Proof of Identification
Driver's license required

Additional Details:
Required Skills/Experience:
- Must be RN Case manager with at least 2 years Acute Care Management experience in discharge planning experience.
- Experience must include at least 6 mos. at a level 1 or large academic Medical Center hospital working with a variety of inpatient units for medical discharge
Preferred Credentials: Would consider ADN
Unit Details: Will cover all Inpatient units throughout the hospital.
Shift & Scheduling:
- Monday - Friday
- Holiday Requirement: Rotated as needed
