Additional Details:

All therapy/rehab traveler start dates will be booked 6 weeks out to provide time to process Marshfield credentialing and onboarding requirements. All travelers will be granted temporary privileges through the Marshfield credentialing team. Marshfield's HR will reach out to the traveler directly alongside our Vaya compliance team throughout the onboarding process to collect these requirements.

PT, SLP, & OT placements will be required to submit the following requirements within 1 week of signing - These documents are required for the credentialing application and failure to complete these timely could result in a pushed start date - DOB, SSN, resume, transcript or diploma, education verification, & minimum of 3 references 2 peers and 1 department chair (need name and email address)

Weekend shifts - Census-based

Weekend Coverage: Every Third

Required: inpatient experience, Bachelor's, Master's, or Doctorate degree in physical therapy

Focus is primarily inpatient will work outpatient as needed - Mon-Fri 3 hrs a day inpatient focused (census based) everything else will be outpatient

Charting System: Any, Required

Orientation Hours: 20

Patient Age Groups: Adolescents, Adults, Geriatrics

If multiple holidays occur during assignment, facility will only accept 1 holiday RTO request

Background initiation, BID, CCAP, and WI caregiver are to be completed and uploaded into Lotus within 48 hours

Current Driver's License or Government issued photo ID required.
