Requirements
Experience
60 months of role experience is required with some in the last 12 months.
Must have at least 36 months of Case Management experience.
Experience with Cerner is preferred from any number of months.
References
Candidates must provide 1 manager/supervisor reference from within the past 12 months in order to be considered for this role
License
Candidates must have a California license

Utilization Review Case Manager

Unit/Facility Details: This multifaceted role will focus on discharge planning, utilization review, and swing bed coordination within our healthcare facility. The Case Manager will play a pivotal role in ensuring the smooth transition of patients from the acute care setting to appropriate post-acute care options, while also managing utilization and overseeing swing bed services.

Required Skills/Experience: A minimum of 3 years’ experience in case management or utilization management, or 5 years’ experience as a registered nurse in an acute care hospital setting.

Required Credentials: Education: Bachelor's degree in a healthcare-related field (such as Nursing, Social Work, or Healthcare Administration).

Special Requests: RTO not approved unless noted in the offer.

Shift & Scheduling: Monday-Friday. Holiday Coverage Required: No.
