Requirements

Experience

24 months of role experience is required with some in the last 12 months.

Travel experience is preferred from any number of months.

Must have at least 24 months of Case Management experience.

3 months of experience with Epic is preferred from any number of months.

References

Candidates must provide 2 references from within the past 12 months in order to be considered for this role

License

Candidates must have a California license

Certifications

BLS (Required)

Vaccinations

Additional Details:

RNs only: Cannot have a license that is on probation, suspended, or has board proceedings that are pending.

Travel pairs are not supported - each clinician must apply and receive an offer separately

Orientation Dates: 10/7 or 10/21

Shift & Schedule: Will work EOW or a set rotation

The set rotation is working every Tues-Sat

Float: Will float between both Walnut Creek and Concord campuses, they are 8 miles apart

Required Skill / Experience: Must have recent Acute Care Hospital Case Management experience

Must have 2+ years experience in an acute care hospital
