Requirements

Experience

12 months of role experience is required with some in the last 12 months.

Must have at least 24 months of Home Health experience with 12 months within the past 3 years.

Experience with Homecare Homebase (HCHB) is required from any number of months.

References

Candidates must provide 2 manager/supervisor references from within the past 12 months in order to be considered for this role

License

Candidates must have a California license

Certifications

BLS (Required)

Proof of Identification

Driver's license required

Additional Details:

Required Skills/Experience: Current HH experience with a Medicare Certified Agency.

Unit/Facility Details: Traveler is expected to have their own transportation.

Shift & Scheduling:

Monday to Friday
Monthly weekend day assignment but no on call.

Floating Requirements: Will fill both West and South territories. West=Russian River area (Monte Rio, Guerneville, Rio Nido, Forestville), Occidental and Sebastopol. South=Petaluma, South Santa Rosa, Rohnert Park.
