Experience

12 months of role experience is required with some in the last 12 months.

Must have at least 10 months of Home Health experience.

Experience with Oasis is required from any number of months.

References

Candidates must provide 2 manager/supervisor references from within the past 3 years in order to be considered for this role

Additional Details:

All positions at Providence Health & Services require that any and all lab items requested during onboarding be completed within 72 hours of signing a contract - no exceptions. This means candidates must be responsive and willing to go to a lab to complete health orders ASAP after signing. Failure to comply may lead to a pushed start or cancellation as assessed by the facility.

References must cover at least 1 full year of relevant experience in specialty within the past 3 years

Required Credentials: Cannot have any hits/suspensions on license or any background hits from any point in the past. Government-Issued Photo ID. Drivers License/State ID. BLS Must be AHA (Instructor, Provider, or RQI) or “Heart&Stroke” certification from Canadian Heart & Stroke Foundation ONLY.

Shift & Scheduling Requirements: This facility cannot accommodate 4x10s or 3x12s.

Special Request Requirements: RTO must be listed at time of submission. Traveler is expected to have their own transportation. System does a good job grouping clients minimizing drive and windshield time. Occasional maximum radius of roughly 60 miles (60-90 minute total drive time) per day.

Unit and Facility Details: Salem OR and Surrounding Areas (South Salem, West Salem, Corvallis, Albany, Sweet Home, Lebanon). Parking Cost: Free.
