Requirements
Experience
12 months of role experience is required with some in the last 12 months.
12 months of Physical Therapy experience is required with some in the last 24 months.
Experience with Homecare Homebase (HCHB) is preferred from any number of months.
References
Candidates must provide 1 manager/supervisor reference from within the past 12 months in order to be considered for this role
License

BLS (Required)
Vaccinations

Additional Details:
The successful candidate will provide in-home therapy services to maximize the independence and safety of our home care and hospice patients.
Valid driver’s license and reliable vehicle.
Experience in a home care setting is preferred.
