Requirements

Experience

12 months of role experience is required with some in the last 12 months.

24 months of Home Health experience is preferred from last 12 months.

AS completion required

Experience with Oasis is required from any number of months.

Experience with Epic is preferred from any number of months.

References

Candidates must provide 2 charge/lead references from within the past 3 years in order to be considered for this role

License

Candidates must have a Michigan license

Guaranteed Hours Work Week: 40

Weekend Rotation Requirements: Every 5th

Holiday Rotation Requirements: Every other

Shift requirement: Depending on census there may be flexibility to start and end times.

On-call: Every 5th weekend with possibility of coverage increasing based on current nursing pool

Travel Requirements: 5 visits per day on average could be up to 6. It can vary depending on the territory, patient census, staff availability, etc. Could be maximum of 150 miles in a given day but not all days.
