Requirements
Experience
12 months of role experience is required from last 3 months.
24 months of Physical Therapy experience is required from last 3 months.
Experience with Cerner is preferred from any number of months.
References
Candidates must provide 2 manager/supervisor references from within the past 12 months in order to be considered for this role
License

Additional Details:
All therapy/rehab traveler start dates will be booked 6 weeks out to provide time to process Marshfield credentialing and onboarding requirements.
The following documents are required to be submitted to compliance within 1 week of signing a contract:
- DOB
- SSN
- Resume
- Transcripts or diploma
- Education verification
- Minimum of 3 references (need name and email address for MCHS to send their reference form) - 2 must be peers, 1 must be the department chair
If assigned to Beaver Dam, an Administrative reference must also be provided
