Requirements

Experience

24 months of role experience is required with some in the last 12 months.

Must have at least 24 months of Hospice experience.

Experience with Epic is required from last 2 years.

References

Candidates must provide 1 charge/lead reference from within the past 12 months in order to be considered for this role

License

Candidates must have a Vermont license or compact license

Certifications

BLS (Required)

Vaccinations

COVID vaccination required. Declinations not accepted.

Additional Details:

Required Skills/Experience: Hospice/End-of-Life care with complex symptom management and patient/family education.

Preferred Credentials: Additional hospice/palliative certifications.

Shift & Scheduling:

• Primary days but will be rotation as we will need coverage on nights. Call coverage rarely needed but could cover on call team.

• Monday to Sunday.

• Holiday Requirements: If needed.

Floating Requirements: May be expected to cross cover adult home health, hospice and community hospice as needed.
