Documentation system: Expanse & careport
Scheduling requirements: Monday - Friday
Floating policy: floats to assignments as needed
Patient Experience requirements: experienced inpatient social worker
Certifications: BSW or higher
Requested Skill Set: Must be familiar with inpatient case management, discharging, guardianship, surrogacy. Comfortable working with families and planning for LTC placement.
No weekends
Must have: candidates without these skills will not be considered for the role.
