Required Skills/Experience Minimum Experience: Home Health experience, training, working with the adult/geriatric population with a variety of orthopedic, neurological and functional mobility diagnosis.
Required Credentials: NPI # required. If the candidate does not have one, they need to start the process of applying for a NPI # in order to start.
Required Education: Master's Degree in Physical Therapy.
Procedures/Diagnostics/Treatments Performed on Unit: Evaluates, treats and plans patient care in accordance with Physical Therapy Standards of practice.
Meets productivity expectations of an average of 4.5 visits per day
Types of Patients on Unit: Population served includes adult and geriatric patients.
Equipment Used on Unit: Cell phone for GPS and communication.
Shift & Scheduling: M-F
Weekend/Holiday: PT's rotate one Saturday every 6-8 weeks. (the Sat. they work would be given a day off during the week) Saturday only. No holiday.
Travelers can be put on call for low census hours. The on-call hours that travelers are placed on will count toward the 36 low census unbillable hours. If the traveler is called back to the facility, they will be able to receive call back pay.
Floating Requirements: Occasional travel to central/ Northern regions
