Experience
12 months of role experience is required with some in the last 12 months.
Must have at least 12 months of Home Health experience with 1 months within the past 6 months.
3 months of experience with Epic is preferred from any number of months.
References
Candidates must provide 2 references from within the past 12 months in order to be considered for this role

Required Skills/Experience:

  • The Occupational Therapist is an excellent opportunity to administer participants' evaluations and therapeutic procedures. This professional-level position offers you independent operation, and the capacity to apply your initiative, analysis, and service orientation in your daily work functions.

Procedures/Diagnostics/Treatments Performed on Unit:

  • Common diagnoses in home health occupational therapy (OT) include neurological conditions like stroke, Parkinson's disease, and Alzheimer's; cardiopulmonary issues such as heart failure and COPD; orthopedic problems including fractures and arthritis; and general deconditioning from aging or falls.

Types of Patients on Unit:

  • Patients may also have diabetes, diabetes-related complications, or wounds that impact their ability to perform daily activities.

Equipment Used on Unit:

  • All clinicians utilize a laptop and phone, both with cellular data, to complete documentation and communicate with team members.

Unit/Facility Details:

  • Beds: Current home health census is 168 as of 9/23/25
  • Visits are 1 on 1 and depending on the visit type, clinician will perform approximately 5 visits in an 8 hour shift.
  • Parking: Varies with each patient

  • Schedule cycle: Mon-Fri
