Requirements

Experience

24 months of role experience is required from any number of months.

Travel experience is preferred from any number of months.

12 months of Case Management experience is required from any number of months.

3 months of experience with Epic is preferred from any number of months.

References

Candidates must provide 2 references from within the past 12 months in order to be considered for this role

License

Candidates must have a California license

BLS (Required)

Must have recent acute care hospital case mgmt exp

Orientation dates: 5/20, 6/3,.

Will float between Concord and Walnut Creek campuses as needed.

Must have 2+ years experience in an acute care hospital.
