Requirements
Experience
12 months of role experience is required with some in the last 12 months.
Must have at least 22 months of Occupational Therapy experience.
Experience with Epic is preferred from any number of months.
References
Candidates must provide 2 manager/supervisor references from within the past 12 months in order to be considered for this role
License

Required Credentials: All positions at Providence Health & Services require that any and all lab items requested during onboarding be completed within 72 hours of signing a contract - no exceptions. This means candidates must be responsive and willing to go to a lab to complete health orders ASAP after signing. Failure to comply may lead to a pushed start or cancellation as assessed by the facility.

References must cover at least 1 full year of relevant experience in specialty within the past 3 years

Cannot have any hits/suspensions on license or any background hits from any point in the past.

Special Requests: RTO must be listed at time of submission.

Shift & Scheduling: Weekend REQ: one weekend day (typically Sunday) every other weekend

Is on-call REQ: No

Floating Requirements: Will this traveler need to float between like units: Acute Rehab and other Inpatient floors.
