Unit/Facility Details: DL/ ID is required with submission. Address on DL/ ID is used to verify local/traveler status

Special Requests: The following dates are black out dates, so please make sure your traveler is aware before submitting 11/27, 12/24, 12/25, 12/31, 1/1

Shift & Scheduling: work weekends, and generally just be very flexible. Weekend Requirement: Yes 1 weekend per month and additional if needed per needs of unit, On Call Requirement: Yes call per needs of unit

Floating Requirements: Yes between the main OR and Outpatient Surgery Center
