Requirements

Experience

12 months of role experience is required with some in the last 12 months.

Travel experience is preferred from any number of months.

Must have at least 12 months of Home Health experience within the past 24 months.

Experience with Homecare Homebase (HCHB) is required from any number of months.

References

Candidates must provide 2 manager/supervisor references from within the past 2 years in order to be considered for this role

License

Candidates must have a Pennsylvania license or compact license

Vaccinations

Additional Details:

Required Skills and Experience: Must complete revisits, SOC/ROCs, recerts and discharges. Case management experience required.

Preferred Skills and Experience: IV and venipuncture experience - Highly Preferred

Required Credentials: MUST have a clear driving record, active driver’s license and current auto insurance. Must provide proof within 48 hours of offer acceptance.
