Preferred 24 months of Physical Therapy experience within the past 12 months.

Candidates must provide 1 charge/lead reference from within the past 3 years in order to be considered for this role

Additional Details:

Required Skills/Experience: Home Health experience, training working with the adult/geriatric population with a variety of orthopedic, neurological and functional mobility diagnosis. Knowledge of Physical Therapy standards and practices acquired through a minimum of a Masters degree in Physical Therapy.

Required Credentials: NPI # required. Required Education: Master's Degree in Physical Therapy.

Shift & Scheduling: M-F 8-530pm, Weekend/Holiday: PT's rotate one Saturday every 6-8 weeks. (the Sat. they work would be given a day off during the week) Saturday only. No holiday.

Floating Requirements: Primary Location: Aspirus at Home – Medford , occasional travel to central/ Northern regions
