Requirements
Experience
12 months of role experience is required from last 3 months.
12 months of travel experience is required from any number of months.
24 months of Home Health experience is required from last 3 years.
Experience with Epic is required from any number of months.
References
Candidates must provide 2 references from within the past 2 years in order to be considered for this role
License
Candidates must have a Iowa license or compact license

Additional Details:
On Call: Y: On-call approximately one weekday per week
Monday-Friday 1630-0800 for PRN visits and one weekend approximately every 4th-5th weekend.
