Requirements

Experience

12 months of role experience is required with some in the last 12 months.

24 months of Case Management experience is required with some in the last 24 months.

3 months of experience with Epic is preferred from any number of months.

References

Candidates must provide 1 manager/supervisor reference from within the past 12 months in order to be considered for this role

License

Candidates must have a Vermont license or compact license

BLS (Required)

Vaccinations

Case Manager – Hospice - Required

Location: Randolph, VT area

Required:

Graduate of an accredited school of nursing

Medical/Surgical/Oncology nursing and/or hospice care experience - Preferred

Shift Information:

Monday - Friday, 8:00 am - 4:30 pm

Holiday Coverage: No

Guaranteed Hours: 36
