Additional Details:

All positions at Providence Health & Services require that any and all lab items requested during onboarding be completed within 72 hours of signing a contract - no exceptions. This means candidates must be responsive and willing to go to a lab to complete health orders ASAP after signing. Failure to comply may lead to a pushed start or cancellation as assessed by the facility.

References must cover at least 1 full year of relevant experience in specialty within the past 3 years

Cannot have any hits/suspensions on license or any background hits from any point in the past.

Required Skills/Experience:

• 2-year acute medical Social Work experience performing both roles of Social work and discharge planning.

• Can screen for manager to review travel experience.

• Case Management/Utilization Review

• Care coordination

• Discharge Planning

• Needs Assessment/ Order DME

• Plan of Care

• Care Coordination

• Collaborative care

• Participates in care conference activities

• Assessment

• Develop plan of care to meet patient/caregiver needs

• Short Term Acute Care Inpatient Medical Social Worker

• Knowledge of drug/alcohol abuse treatment options, patients experiencing homelessness, patients with mental health history or in crisis, advance directive, POLST, protective services process, etc.

Required Credentials: LCSW either/or MSW, MSW.

Special Requests:

• RTO must be listed at time of submission.

• Anything more will potentially delay your offer while we run approval by the client. To minimize turn around please limit total RTO requests

• Must commit to working 3 of the 5 holidays (Thanksgiving, Xmas Eve, Xmas Day, NYE, NYD).

Shift & Scheduling:

• Weekend requirement: typically one full weekend every 4 weeks, subject to departmental needs.

• Weekend Coverage: Every Other

• AUTO OFFER - NO INTERVIEW: This is an auto-offer position. You will likely not interview with the facility prior to receiving an offer. However, an interview can be arranged upon request. All time off requests must be submitted at application. Scheduling requests must be worked out with the manager directly upon starting the assignment. Any offers for this job will require a clear "Yes" or "No" response from the nurse within 24hrs or the offer must be declined.
