Requirements

Experience

12 months of role experience is required with some in the last 12 months.

Must have at least 12 months of Physical Therapy experience within the past 3 years.

3 months of experience with Oasis is required from any number of months.

Experience with Homecare Homebase (HCHB) is required from any number of months.

References

Candidates must provide 2 manager/supervisor references from within the past 2 years in order to be considered for this role

License

BLS (Required)

Vaccinations

COVID vaccination required. Religious and medical declinations accepted.

Additional Details:

Float experience does not count towards the unit experience requirement.

Required Credentials: Client may consider without Homecare Homebase if CLN agrees to complete online HCHB course prior to start date.

Shift & Scheduling: Monday - Friday. On call is required.

Floating Requirements: Coverage: St. Paul and surrounding; Roseville, St. Anthony, St. Paul, West St. Paul, South Minneapolis, Lilydale. May be asked to help in other territories within facility's service area as needed which is about a 60 mile radius from the Facility address.
