Requirements
Experience
12 months of role experience is required with some in the last 12 months.
Must have at least 3 months of Home Health experience within the past 3 years.
Experience with Homecare Homebase (HCHB) is required from any number of months.
References
Candidates must provide 2 manager/supervisor references from within the past 2 years in order to be considered for this role
License
Candidates must have a Oregon license
Certifications
BLS (Required)
Vaccinations
COVID vaccination required. Religious and medical declinations accepted.

Additional Details:
Float experience does not count towards the unit experience requirement.
Required Skills/Experience: Must have recent HH exp 2023-24.
Unit/Facility Details: Client may consider without HCHB exp if CLN completes online training class prior to start.
Shift & Scheduling:
• Monday - Friday
• On call one weekend per month.
Floating Requirements: Coverage area: Gold Hill to Grants Pass and south to Williams and everything in between
